2 Full-time 1 day ago Full Job Description The Patient Care Optimization LVN is responsible for collecting patient data, performing new patient referral procedures, and identifying appropriate candidates for home care services.
This position is responsible for accepting and processing all referrals from patients, families, hospitals, physicians, and other referral services when requested.
The LVN executes tasks within the Admissions department as directed and within the LVN Scope of Practice.
This includes the completion of required documentation and processes to ensure that start of care is initiated in a timely manner according to regulatory guidelines.
Demonstrates and provides excellent customer service to referral sources, patients, and family members.
The LVN is aware of internal and external services, facilities, and community resources, and develops and maintains rapport with members of the medical community.
Monitors the progress of the referral through the referral process, and reports to the appropriate supervisor and team when changes in information, and data received may impact the intake and referral process.
Salary Range:
$34.
65 - 38.

Qualifications:
Possession of a valid and current license to practice as a Licensed Vocational Nurse in the State of California.
