Demonstrates knowledge of the principles and practices of speech pathology and provides speech therapy services in accordance with state practice act Performs an initial assessment and evaluation of patient and in establishes a plan of care, including scheduling subsequent visits determined by the SLP to be clinically appropriate.
Coordination of care by case conferencing with other health care team members communicating barriers to achieving goals as indicated Observes, records and reports to Primary Care Physician the patient's treatment, response and progress including significant changes Obtains orders to revise plan of careas clinically appropriate Performs re-evaluations of patient's progress at least every thirty (30) days and updates plan of care as necessary.
Completes comprehensive assessments and OASIS dataas necessary Accepts patient referrals from other disciplines as well as primary care physician for appropriate impairments including swallowing dysfunction, speech and language production and cognitive rehabilitation as clinically appropriate Teaches patients and their caregivers about the patients' impairments and provides support and educational materials to support interventions and foster independence.
Minimum Requirements:
Master's degree in Speech Language Pathology Active Speech Language Pathology license (CCCs) Ability to practice speech therapy unencumbered within state and federal guidelines At least one year of experience working as a Speech Language Pathologist (no CFYs) Must have vehicle, current driver's license and appropriate automobile insurance.
Ability to make home visits and provide speech therapy evaluations and treatments in the home environment complete documentation effectively and submit in a timely manner.
Preferred Requirements:
Home health experience Home Care Home Base documentation experience Essential qualities and attributes:
Strong critical thinking and problem-solving skills Excellent written and verbal communication skills Team attitude Flexibility.
Estimated Salary: $20 to $28 per hour based on qualifications.
